All Patient Refined Diagnostic Related Group (APR-DRG)

Elke APR-DRG wordt in vier klassen van klinische ernst (SOI, Severity Of Illness) en vier klassen van sterfterisico (ROM, Risk Of Mortality) onderverdeeld. De toekenning van een klasse van klinische ernst alsook van een klasse van sterfterisico geschiedt in verschillende fasen:

• 1 = zwak
• 2 = matig
• 3 = majeur
• 4 = extreem

Voor toepassingen zoals de evaluatie van de gebruikte middelen of de uitwerking van richtlijnen moet men de APR-DRG classificatie gebruiken plus de graad van ernst (SOI). De APR-DRG's samen met de onderverdeling op basis van het sterfterisico dienen voor het analyseren van de geobserveerde sterftegraad (ROM).

Om de werkingsmiddelen voor de financiering verder te verfijnen wordt een APR-DRG in België onderverdeeld in leeftijdsklassen (statistische basis):

• < 75 j (SOI 1 en 2) = L
• > 75 j (SOI 1 en 2) = H
• A (SOI 3 en 4)
• G en Gfin (SOI 1, 2, 3, 4)

Voor de patiënten met SOI 1 en SOI 2, wordt een onderscheid gemaakt tussen die jonger dan 75 jaar, vanaf 75 jaar en een bijzondere groep van geriatrische patiënten of Gfin genoemd. Voor SOI 3 en SOI 4 wordt enkel onderscheid gemaakt of de patiënt tot de Gfin groep behoort of niet.

Om tot de Gfin te behoren moet de patiënt verblijven op de geriatrische dienst van een ziekenhuis waarop de gemiddelde leeftijd 75 jaar of ouder is of moet de patiënt zelf minstens 75 jaar oud zijn. Daarenboven moeten er voor de patiënt minimaal 10 verblijfsdagen in een bed met kenletter G gefactureerd zijn en dient de verblijfsduur minstens 30% langer te zijn dan de gemiddelde nationale ligduur voor een hospitalisatie van een patiënt van 75 jaar of ouder die is ingedeeld in dezelfde pathologiegroep met dezelfde SOI.

Verder nog in verband met de impact van G reëel (GR) en G Potentieel (GP, GPot) op de financiering. Een verblijf wordt aangeduid als een reëel G verblijf indien:

• de patiënt minimum 70 jaar is
• de patiënt minimum 2 aangetaste systemen heeft
• het verblijf een totale ligduur heeft die hoger is dan het gemiddelde Gfin x 0,5
• de leeftijdscategorie verschillend is van G
• het aantal gefactureerde dagen in G groter is dan 0
• de patiënt behoort niet tot categorie 9 (de foutieve verblijven)

Verder is er nog de beperking van de G dagen van de GR (G reëel) GP (G Potentieel) patiënten tot 6 bedden Voor de GR en GP-patiënten wordt er een aantal G-bedden toegekend, ten belope van maximum 1/4 van de capaciteit van een complete eenheid G-bedden (namelijk 6 bedden). Het overschot van ligdagen (> 6 bedden) wordt verschoven naar bedindex CD.
